State Forms
Once you have registered and your kiddo has been accepted into a class, the following State of Ohio documents need to be filled out and returned to us at littlelionsps@gmail.com before the first day of school.
If you selected YES to an question on Form DCY-01234 that stated you needed to fill out additional forms, these forms are provided below. Otherwise, you DO NOT need to complete these forms.
